Case Management at Wendell Foster helps individuals and their families navigate their journey with confidence and support.
Using a Person-Centered Planning approach, our Case Manager works hand-in-hand with each person to coordinate care, connect them to valuable community and waiver resources, and ensure their needs are fully met. The Case Manager serves as a trusted advocate- someone who listens, guides, and helps safeguard personal choices every step of the way.
Case Management services are available through the Supports for Community Living (SCL) and Michelle P. Waivers, as well as on a fee-for-service basis.
